FEMA Aid Application Checklist

  • Social Security number for an eligible household member
  • Name and basic personal information for the applicant
  • Information about the household member who meets FEMA’s citizenship or immigration eligibility requirements

2. Contact information

  • Address of the home or property damaged by the tornado
  • Current mailing address, if different
  • Phone number where FEMA can reach you
  • Valid email address

3. Insurance information

  • Types of insurance coverage you have, such as homeowners, renters, flood, mobile home or vehicle insurance
  • Insurance company and policy information, if available
  • Insurance claim information or settlement documents, if available

4. Description of the damage

  • Type of disaster and date of damage
  • Type of property affected, such as a house, apartment, mobile home or vehicle
  • Description of damage and disaster-related losses
  • Photos of damage, repair estimates and receipts, if available

5. Household income

  • Total annual household income before taxes at the time of the disaster

6. Bank information (optional)

  • Bank name

Winnebago County Tornado Event Information & Resources

County-level hub. Includes the storm damage viewer built from July 29 drone imagery covering Fox Crossing, Menasha and Neenah, which residents and businesses can use to document damage. The county’s Multi-Agency Resource Center has also moved online here.
